The Lived Experience Hub is part of the Coming Home Action Plan. The plan aims to ensure that people with learning disabilities are only ever in hospital when there is a clear clinical need and that everyone receives the appropriate support to live in their chosen communities.

The Hub will be a forum for people with learning disabilities and their families to lead and drive change, helping to shape and influence national workstreams and inform decision making as well as providing opportunities for family and peer support.

Membership of the Hub is open to all people with learning disabilities and their family members with experience of

• living in hospital settings long-term

or

• having to live away from their own community in an “out of area placement”

or

• whose support package is at risk of breakdown.
